The rail is not associated to a work plane and thus it moves depending on the thickness of the wall. You can edit the family to fix this:
Select the rail, change the associated work plane to Back, then change the extrusion to -2" and -2 3/4". This should keep all of the towel bars looking the same in plan no matter the thickness of the host.
