Anyone have any trouble with the little plus sign buttons in the project browser disappearing? It happens every day to me and I am getting tired of it. The categories still expand when I click on the area where the plus buttons should be, but it's a little annoying when you don't know which items are expandable and which are not.
The only way I can get the m to come back to to completely shut down Revit and restart.

I will guess that you are using a theme in your windows (?)
just go back to "windows classic theme" and your + will be back
that's what I found out with my machine...

Nope, no "theme" here. I have it set to the standard XP theme and only changed the background to black. Any other ideas?

I have had this problem as well, with no custom windows themes active. It doesn't happen everyday, but it is an annoyance when it does.

It is the theme. You have to restart Revit after for it to take effect. In your display properties in Windows make sure that Theme is set to Windows Classic. Dont know why it does that, and your right it doesnt happen every day.
