So, I am from Europe and I don't know how they do it in the rest of the world but over here it's customary to have external doors and windows sit semi-flush with the wall, and swing inwards.
However, I find that this is not how Revit likes to do it. When you switch swing direction to let a door or window swing inwards, the door or window also moves inside the wall to sit flush with the inside face instead of the outside face.
There are some pictures attached to this post to make things a bit clearer, at list I hope they do.
Anybody any idea how I can make this work? I also have a few tilt-turn windows that give me the same behavior.

I am sorry, forgot to mention the Revit version I use. I use Revit 2018.

A standard parameter you should have for all butt frame windows and doors in the family is 'Inset from Exterior'. To accomplish this in the family create a reference plan within the host wall in the family and dimension from this to the reference plan which is locked to the face of the wall. Then all other parameters within the family that currently dimension to the reference plane on face of wall or sketch elements that are locked to this need to be reassociated to this new reference plan. Now with this new parameter you can adjust the positioning of the door/window within the wall.
If you flip the door so it swings in you just need to figure out the ditance to offset so that the "back edge" of the frame is flush with the wall face you want.
The main reason for this parameter in my experiene is so that the door panel/window glazing can be aligned with the exterior wall rigid insulation lines as this location varies based on cladding (brick, siding, stone, etc).

I agree with marmketin. While still working, I created all of my offices door families to coreect ills like you describe. All of my doors had reference planes they were built around that could control where to were in the wall in relation to the controlling face. Flipping the swing made no differences.
I had over 30 different door types, all with hardware options and different adjustable frames.
Lots of work but well worth developing. I did not bring them home when I retired because I did it on company time.
