I am putting windows in a wall and at first they did not show up in the floor plan view. You would see the wall structure, but there would be no window until you moved your mouse over the spot where it was, then you could see the highlighted window. Today, I am able to put the same window into the wall and see the window in the wall now, but if I change the sill to above 3' 11" height, the window disappears. Below 4', the window shows up normally. I can see the window just fine in 3D view, but not in floor plan view. The wall just continues like there is not a window there.
I am also having a similar problem with a door. Where the door swing will not show up on one side of the wall and I have to have the door open the other way to show the door swing.
ALSO: I'm starting to see elements such as doors show up in alternate floor plan views. For example, I put a garage door on the 1st level and it shows up in the foundations as well. Same thing is happening with ordinary doors. They are set to 1st level, but they are showing up in foundations level as well.
I was really thinking I was getting Revit down and now this! This is madness! Help greatly apprciated.

Revit uses the view range cut plane that is associated in the family for how it shows in the project. Your issue is likely that your cut plane in the family is set to 3'11" and so in the project if your view range cut plane is above this number even if the window should be cut it will not show this way. What you need to do is set the view range cut plane in the family to something like 5' as pretty much all windows will start below this and and above this.
