Does anyone know if it is possible to override the dimension text and substitute (or add) a word -- such as "Equal" or "Clear" or O.C.?

You can add a suffix or prefix to a dimension. Just check the properties box of the dimension to add.
You cannot however "over-ride" a dimension. This has been dicussed ad-nausim on this forum and on AUGI. You can place a piece of masking style text over the "dimension" or I will submit my 2 cents that then what you have is not a "dimension" at all rather an annotation.
If you want things to be "equal" then click the "eq" blue text of the dimension string and Revit will space the items equally and replace the dimension to report "Equal".
