I have created an annotation familly which have 4 arrows in 4 directions (x,y), And I want to choose, just like with the elevation family, which arrow will be shown with check boxes. Does anyone how can I do it?

An instance parameter in the family that is yes/no will give you a checkbox in the properties window

Great, that helps! Is that anyway I can have the checkbox on the screen close to the symbol. Like in the attachment.
