When adding revisions to a titleblock i can only show one revision per sheet, when a add a second revision to the sheet the origional revision dissapears and no revisions are shown in the titleblock but both clouds are visible... any good ideas?

Are you sure that your revision clouds didn't accidentally get turned off? If they are off, then your revision description in the title block will turn off also. Go to the bottom of the screen and click your reveal hidden elements

You don't add revisions to a titleblock. The revisions are added when you place a revision cloud on the sheet or in a view on the sheet.
It sounds like your titleblock was not created correctly.
