I have created a topo plan phase=existing within revit2015 from scratch, drawing over an overlay. I have also inserted a building pad, phase=existing. all good, looks fine. next steps to proceed to new building addition (already drawn phase=new)
- create graded region
- within a siteplan (set to phase=new): create a new toposurface/1st option=create and exact copy
- immediately, it reports that we have cut and fill, which is not the case because we only have an existing plan http://screencast.com/t/LjxHBhzU
[1] shouldn't this new topo plan have zero cut and fill in the new phase (since I am just starting)? I suppose I can just start from these (wrong?) numbers, make a note of them and adjust the "new" cut and fill numbers by those ammounts.
[2] each time you want to work on the topo - do you create another new topo-region copy of the existing topo? Or just modify the original one?

First, I think topography doesn't play well with phases so new/exist doesn't matter.
I don't understand why you put a building pad in existing phase. If this is an existing building area, then why not just a hole in your topography for the existing model? There then is no reason for a new topo. Just grade the regions and add your new regions.

It's an existing buidling (with an existing buidling pad). The addition is a "new" phase pad.

You really don't ever need a PAD under existing work. Our slabs are layered and include the granular fill. Thus our pads have no thickness and really just cut a whole in the topo. True ... they will still show earth poche under the slab but that is all. You can get the same affect by splitting the topo then by editing the resultant topo and chaing all of the points elevation to the elevation at the bottom of your slab/aggregate floor. Now - everything can still be existing and there is no cut.
