I created a custom symbol that i have been using as a keynote workaround. I am currently trying to make a leader line to indicate where the keynote is, but the leader starts in the top corner "ish" of the hexagon instead of at the point. see image. any ideas on how to fix/workaround would be great. Thanks.

When ever i do keynotes it never comes up as i want so instead i just create a text box populate it with the notes, then use symbols with leaders in the drawing. it has worked up until today.

Wow - why not use CAD?
Sorry but you need to learn to use the tools that Revit has. Your "work around" is harder - not easier! Keynotes work and are easy to do. Watch some videos and ask specific questions here when you get stuck.

I have looked into the keynotes and they don't work like i would like them to. I need different keynotes per page and every time i create a key note it uses the text file linked to the project. So when I want keynote 1 on page 1 to say "one thing" then page 2 keynote 1 to say "something else" it doesn't work. Page 2 keynote 1 says the same thing as page 1 keynote 1. That's why i am using my "work-around". If you have any ideas on how to fix/get around the "issue" differently than i have been maybe you could share it, or even share a link to these videos that you are talking about.

You have two keynote setup options. Numbered by keynotes or numbered by sheet. It sounds like you want numbered by sheet. With that, the keynotes are numbered according to when you place them. Keynote 1 is the first one used and it can be different on every sheet.
You will find the keynote settings under the annotation/tag dropdown - keynote settings.
