Developing a new project in version 2013, and four new phantom levels are appearing out of nowhere, labeled: Level 3 T.O.W., Level 3 T.O.X., Level 3 T.O.Y., and Level 3 T.O.Z. Three of them are located on the lowest level, and the fourth appear on the third. None of which appear in the project browser. Has this happened to anyone?

If they are not associated with a floor plan they will not be in your project browser, they are simply just levels.  Is this a new project, did you use a project template to start the job that may have the levels in them?  If you don't want them there just erase them.  Somebody made them either in your project or in the project template file that you used.

The levels show up in multiple elevations, meaning they are somewhat 3-dimensional. The project template did not start out with them. I deleted them, no big deal, but then another Level 3 T.O.AA appeared. Cursed Revit file?

All levels are 3D and they are ones that you created or ar in your template or are in a Revit file that you linked in.
