If you wanted to create a new system family, how would that be done? By duplicating an existing family or by using Revit family templates? (only one of these answers is correct - and I don't know which one)

You can not create system families - only autoCAD can do that. (That is why there are no templates for system families.)
You can create new system family types. You do that by duplicating a system family type while in the project (or template) and define that new type.
