Is there a way to rotate a viewport on a sheet without also rotating the title mark of the viewport? Or at least is there a way to rotate the title mark back into postion after the viewport rotate?

I go to Tools... Project Position/Orientation and Rotate True North. Then if you go to View Properties you can orient a project by Project North or True North.

Rotate the title;
Open the View Title family and rotate it in the family - rename (Save As) and then load into your project.
Select the view title - go to properties - Edit / New - Duplicate and assign the "Title" parameter to your new family.

How do you get the viewport itself to rotate? Does the extension line rotate as well?
It looks like you can also rotate views if you establish a crop region for the view.

It doesn't appear to, but you can toggle it off in the type parameters and draw one in.

EASIEST WAY:
PLACE VIEW ON SHEET.
ACTIVATE THE VIEW.
SELECT THE CROP REGION.
ROTATE THE DESIRED AMOUNT.
NOTE THE VIEW ROTATES AND THE CROP REGION REMAINS THE SAME.
THIS ALLOWS YOUR VIEW TITLE TO REMAIN HORIZONTAL.

That's for plans... if for some reason you wanted to rotate sections or elevations you'ld have to use Rythmick's solution (view rotation on sheet.. 90 clockwise - haven't figured out counterclockwise yet because the view title text isn't cooperating).
Legends are easy enough to rotate, haven't figured out if you can rotate schedules. One would probably only need to rotate plans anyway.

You need to create a third View Title family, rotate the original clockwise 90 deg and in each one of the label properties toggle off the "Keep Readable" box.

I guess maybe for presentation drawings you may need to rotate elevations. But in construction documentation i don't see a need...
Couldn't you just rotate the titleblock?
