You need to create a new baluster family for the posts. Do this using the baluster template where you create the post with the pin on top and the connections at the bottom. There is some guidance on creating balusters in the families guide. Load it into you project. After you have created the stair, click on the balustrade, duplicate it and give it a new name. In the properties, under balusters, load in the baluster that you have created and set the spacing. Uncheck baluster per tread. You will need to experiment a bit to get the spacing correct using the different options. For the railing you will need two profiles, one for the handrail and one for the steel wires. In the browser, go to families, profiles and find the circular handrail. Duplicate it and create a profile with the diameter of the handrail and another with the diameter of the wires. In the railing properties, under rails structure, insert the profile for the handrail. This will be used to determine the top of the balusters. Insert profiles for the wires at the appropriate heights. The curved bit of rail at the end will probably have to be modelled separately. You can do this as an in-place family using the generic template. Hope this gets you started. There are many options for creating railings and they can be quite complicated but quite a lot can be achieved. There is a file with different types of railings that have been created on the Autodesk site, although I don't have a link to it at the moment. Perhaps you can find one that's quite close to what you need and modify it to suit.

One thing to remember about creating unique railings is that you can have more than one railing in the same place. In this case, you could have one railing with the lower rails and large diameter ballusters and another with the top rail and the small diameter balluster top. The ends could be a post that forms the curved handrail extension.

Another thing is that balusters can be placed in the same position if you set the 'Distance from previous' as 0. So you could have the large diameter as one baluster and the top pin as another baluster in the same place, setting in the relevant hosts and offsets for each one.

When you created the the baluster there was an option to 'Load into project'. If you did that then it's there. If you didn't, go to 'Load family', under Insert, find it and load it. In the railing element properties, under balusters, you will be able to find the baluster in the drop downs under Baluster family.
