I was doing a monolitihic stair with railing but encounter a serious problem. As attached image shown, my stair railing does not land properly but it is suspended in mid air. My stair riser height is 167mm and thread depth is 255mm. My floor to floor height is 4000mm. Where did my stair settings possibly goes wrong? One more thing is that the stair goes all the way to the top where the top of the stair is at the top of the floor. Should the design be that the staircase actually stops 1 riser before the floor. Would appreciate suggestion ASAP. Thanks.

You need to edit the point where the railing "breaks" from sloping to flat. If you pull this point back "down" the slope, the flat railing will move down. You should be able to idendify this point in elevation and mark it with a reference plane. Then edit the railing in plan and pull this juction of lines back to that plane .. then discard the plane.

For your second question, there is an option in the stair properties to end with a riser.
