If you hide a cloud, the revision will go away. So for your non revision revisions, place the cloud on the sheet and only turn off the show cloud setting in the setup..... DON'T hide the cloud.

The clouds are not hidden. I tested all "None", "Tag" and "Cloud and Tag" on the Show column. Still not working.
I also notice that even though Seq #27 description is not appearing, but the schedule is recognizing the revision. The description space is left blank. Othe subsequent revisions will appear above it. But the schedule is not recognizing Seq #38 & 39. Even if I manually input in the descriptions, subsequent revision descriptions would overlap on top.

Obviously I am at a disadvantage because I don't have your model but the system works....when done correctly. Verify this. Turn on the clouds for the revisions in question. Then one at a time, click on the cloud and look at the properties. 9 times out of 10, the cloud is not on the revision you think it is. To change it, you have to temporarily change the "issued" status.
Remember, in order for the revision to show in a schedule (even a revision without a number), there has to be a cloud.... we prefer it on the sheet BTW.

I verified all the clouds and they were on the right revisions. I tried testing some other variables. And very strangely, I edited the revision schedule to sort the revisions by date (it was sorted by number then alphabet before) and reloaded it again to the project, THAT fixed the problem!

Great..... I would not have come up with that answer but I'm glad you did because posting it here may help others.
There are so many possibilities in Revit and without seeing the actual problem first hand, it is often very hard to help.
