DESIGNING A HOSPITAL ALSO, WE HAVE SEVERAL ALTERNATES, INCLUDING SHELLED SPACES, DIFFERENT USE AREAS, AND WHETHER OR NOT THEY WILL INCLUDE THE HELIPAD WE HAVE DESIGNED IN THE BASE BID.  USED DESIGN OPTIONS FOR THESE, BUT AS WWHUB SAID, THEY ARE TRICKY AT FIRST.  I FOLLOW THESE STEPS:

A) WHEN YOU WANT TO ADD SOMETHING TO A DESIGN OPTION: IF THAT SOMETHING EXISTS, THEN YOU CAN CLICK ON THE LITTLE "+" BUTTON ON THE DESIGN OPTIONS MENU BAR AND ADD IT THAT WAY, IF IT DOESN'T EXIST YET, THEN EDIT THE DESIGN OPTION YOU WANT TO ADD THE OBJECT TO. 

B) RULES FOR DESIGN OPTIONS:  IF AN OBJECT IS GOING TO BE IN A DESIGN OPTION, AND IT IS A HOSTED OBJECT, THEN IT'S HOST ALSO NEEDS TO BE ADDED TO THE DESIGN OPTION.  THIS CAUSES A LOT OF "DOUBLE" WORK AS IF YOU WANT A DOOR IN ONE OPTION, BUT NOT IN ANOTHER, YOU HAVE TO DUPLICATE THE WALL, BECAUSE THE DOOR HOSTS IN THE WALL...

C) I WAIT UNTIL MOST OF THE ANNOTATING AND DETAILING IS DONE ON THE BASE BIDS, THEN I JUST DUPLICATE THOSE VIEWS WITH DETAILING.  GO TO THE NEW COPIED VIEW, RENAME IT, AND IN VIEW PROPERTIES, CHANGE WHAT OPTION IS VISIBLE IN THE VIEW TO THE ALTERNATE BID THAT YOU WANT TO  SEE.

LAST AND FINALY, BE CAREFUL!  ONCE SOMETHING IS PLACED IN A DESIGN OPTION, IT IS

STUCK IN THAT DESIGN OPTION (UNLESS SOMEONE ELSE KNOWS BETTER?).  TO REMOVE FROM A DESIGN OPTION:  EDIT THAT DESIGN OPTION, CTRL+C (COPY TO CLIPBOARD) THE ELEMENT, THEN DELETE IT.  EXIT THE DESIGN OPTION, GO TO THE OPTION WHERE YOU WANT THE ELEMENT TO BE RESTORED TO, THEN PASTE IN SAME PLACE. (YES, THIS LOSES ANY DIMENSIONS, OR ANNOTATIONS ASSOCIATED WITH THE OBJECT IN ANY AND ALL VIEWS IN WHICH IT WAS ANNOTATED, THUS WHY I SAY BE CAREFUL). 

ONE LAST THING, BUILD A TESTFILE, PLAY AROUND WITH DESIGN OPTIONS A BIT, TRY TO DUPLICATE WHAT IT IS THAT YOU WANT ACCOMPLISHED, ON A SMALL SCALE TO ENSURE YOUR UNDERSTANDING, AND THAT IT ACCOMPLISHES WHAT YOU WANT.

Activate the DO, then cut the wall. Get out of the DO and paste/aligned, current view in the main model.
